Consumer Rights and Instructions

Luz de Luz Counseling, PLLC

In accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105, Luz de Luz Counseling, PLLC, provides the following information to assist consumers in:

• Requesting their health care records

• Contacting the applicable licensing authority

• Filing a consumer complaint

1. Requesting Your Health Care Records

You may request copies of your records at any time. Requests must be made in writing, and Luz de Luz Counseling, PLLC, will provide copies or a summary of records within 15 business days in accordance with Texas Health & Safety Code §611.0045. Reasonable fees may apply for the preparation and delivery of records.

Current and former clients may request copies of their counseling and mental health records maintained by Luz de Luz Counseling, PLLC. Requests must be submitted using the Authorization for Release of Information available through your client portal.

To help prevent delays, please include the following information with your request:

  • Client’s full legal name and date of birth

  • Current phone number and/or email address

  • Description of records requested, including specific dates if applicable

  • Client signature and date

If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.

Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.

2. Contacting the Licensing Authority

Luz de Luz Counseling, PLLC is regulated by the Texas Behavioral Health Executive Council (BHEC), which is the licensing and disciplinary authority for marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.

Consumers may contact the Texas Behavioral Health Executive Council with questions regarding licensure or professional regulation using the following methods:

3. Filing a Consumer Complaint

Consumers may file a complaint with the Texas Office of the Attorney General – Consumer Protection Division using the following options:

When filing a complaint, consumers are encouraged to include relevant dates, names, and any supporting documentation, if available.
